How to Apply and Contact Us

Thank you for your inquiry regarding the Indiana University School of Medicine's Pediatric Pulmonology Fellowship Program. We are currently accepting applications for July 2013. When applying, please complete the application in its entirety and submit:

1. Application
2. Three (3) letters of recommendation from physicians familiar with the applicant's medical training, (one of these must be from the Director of the applicant's Residency Program)
3. One copy of the applicant's medical school transcription
4. A one page personal statement
5. A current curriculum vitae.

All applications should be submitted through ERAS.

Please note that at this time, we are able to consider applications from U.S. residents only.
