Asthma Screening Tools - English
• Ages 2-4 years – Test for Respiratory and Asthma Control in Kids (TRACK)
• Ages 4-11 years – Childhood Asthma Control Test (C-ACT)
• Ages 12-18 years – Asthma Control Test (ACT)
Using validated clinical scoring tools was selected as an intervention for this project due to the ISDH’s high estimate of poorly controlled pediatric asthma in our state (75% in 2013), the existence of brief, easy to use validated clinical screening tools for asthma symptoms, and the anecdotal assumption that improved detection of poorly controlled asthma will lead to increased appropriate escalation of asthma management therapy.

Asthma Action Plans
Asthma action plans are an important tool of communication between providers, families, schools, and subspecialists. This intervention was selected because the annual completion of an asthma action plan is a recommendation of the NHLBI, and is an important marker of clinical quality and patient/family education.
